All on four or all on six?

Full-arch teeth restoration methods using the "All-on-4" and "All-on-6" protocols have become a true breakthrough in modern dentistry. They allow for the complete restoration of the entire dentition in cases of full or near-full edentulism (missing teeth) within a short timeframe, combined with the immediate loading of a fixed prosthesis. Today, these advanced technologies are actively utilized at SHEDO Dentistry and are selected individually based on each patient's specific clinical situation.

The History of "All-on-4" and "All-on-6" Implantation

The concept of full-arch prosthetics supported by a limited number of implants emerged as a solution to severe bone tissue atrophy and the reluctance of patients to undergo complex bone grafting surgeries. The "All-on-4" method involves the placement of four implants, two of which are inserted at an angle to maximize the use of available bone volume. Later, the "All-on-6" configuration was developed, providing even more even load distribution and enhanced structural stability, especially under heavy chewing loads.

Four or Six Implants?

The choice between four or six implants is always made by the doctor based on comprehensive diagnostics. Factors such as bone tissue density and volume, jaw anatomy, bite (occlusion), the patient's age, and functional requirements are carefully evaluated. The "All-on-4" technique offers advantages such as less invasive surgery, reduced treatment time, and a more affordable cost. However, in cases of low bone density or excessive occlusal loads, it may be less optimal. On the other hand, "All-on-6" provides greater stability, reduces the load on each individual implant, and is often chosen for a highly predictable, long-term result. Its downsides may include a more complex surgical procedure and an increased number of implants, which impacts the overall cost of treatment.

All-on-4/6: Sheleg and Dobritskiy

A significant role in the development of full-mouth restoration techniques in Eastern Europe was played by SHEDO specialists Vladimir Sheleg and Anton Dobritskiy. They were among the very first to implement the "All-on-4" and "All-on-6" protocols in Belarus, adapting them to handle complex clinical cases and patients with severe bone atrophy.
